Below is a declaration of an Accredited Investor (“משקיע מסווג“) according to the Israeli law. Please mark one of the sections below to declare that you are an Accredited Investor.

For this section:
“Liquid Assets” means Cash, Deposits, Exchange traded securities, Bonds, Options, Futures, Structured Products, Index Products, Units or Shares in Mutual Funds in Israel or abroad, “קרנות השתלמות“, and equity in private companies.
“Family’s gross income” means Individual and his dwelling family members.
